COVID-19 Vaccine Information

We are pleased to offer on-site COVID vaccine clinics for all students ages 16+ (and 12+ if approved) with a signed consent form on June 9th from 10:00 am until 6:00 pm. The clinics will be held at Biddeford High School in the Tiger Gym.  If you choose to have your child(ren) vaccinated, please carefully review the details below.

APPOINTMENTS: 

  • The timeframe of 10:00 am - 2:00 pm  is exclusively reserved for BHS students having the vaccine while in school.  All other times are open for appointments to BMS and BHS students as well as employee’s children.

  • Students 12+ should register now in the event it is approved prior to the clinic dates.

STEPS TO SIGN UP:

After reviewing the following links (Pfizer information sheet, pre-vaccine checklist, and post-vaccine care sheet, you will need to complete the three steps below to sign your child up for this vaccine clinic:

  1. Complete this Registration Form (linked).

  2. Complete this Consent Form and submit electronically to vaccine@biddefordschools.me  

    1.  Paper copies will also be sent home and should be returned to the School Nurse.

  3. Click on Sign-up Genius here to choose a timeslot for your child’s appointment. 

    1. Reminder: BHS Parents who opt to have their child vaccinated while at school should choose the appointment slot of June 9 from 10:00 am - 2:00 pm and BHS staff will coordinate the ideal time for your child to attend the clinic based on their class schedule.  Please ensure your child is aware you have signed them up to receive the vaccine.

The COVID-19 vaccine is provided at no cost. A fee for administration of the vaccine will be billed to your insurance carrier with no out-of-pocket cost to you.  If you are accompanying your child, please bring a copy of your insurance card.  Your child will not be denied the vaccine if they do not have insurance.
